CoRDS Enrollment Instructions for the 

TGA Project Collaboration

NOTE:

Your computer may have a pop-up blocker to authorize or to continue.

 

VIEW:

CoRDS Screening Form has bright blue accent bars, radio buttons, fill-in-the-blank, and drop-down menus.

 

NOTES:

1. You will be answering: Basic Personal Info, Diagnosis Info, and Other Info.

2. Birth Country - use their search icon to find your country.

3. Diagnosis Box - type in Transient Global and a pop-up should appear, use the right arrow on the screen to move Transient Global Amnesia into Selected Disease Box.

4. How did you learn about CoRDS? Type in TGA Project.

5. Select a Language can be skipped if you checked Yes in the previous line or use the drop-down option.

6. Your option: subscribe to their newsletter or not.

7. I suggest Preferred Enrollment by email, but you can select postal mail.
